Diabetes Health Plan (DHP)

The UnitedHealthcare Diabetes Health Plan (DHP) is an employee benefits offering that supports diabetics, pre-diabetics and their family members. DHP integrates a series of condition-specific features members with the cost-saving, self-management aspects of a standard UnitedHealthcare plan design, such as Choice Plus or Consumer Driven Health Plan (CDH). DHP is an add-on that is wrapped around, or bundled, with those plan designs.
