Standard Care Coordination

Standard Care Coordination (SCC) provides members who are facing a health situation with much needed support through health care professionals, clinical programs and information. Standard Care Coordination encourages wise health care decisions by providing employees with expert advice and clinical resources.

Assistance is offered to employees and their family members at several points of contact:

  • Nurse team is available to answer questions about health concerns, educate employees on available resources and inform them of other services that may be helpful. Members experience continuity in their care by connecting with the same case manager from the start of a health event until it is completed. A nurse may reach out to a member if the member is:
    • Determined to be “high risk” for a chronic or complex health condition
    • Experiencing a health issue that requires extra attention
    • Hospitalized
    • Receiving an organ/tissue transplant
    • Identified as someone who may benefit from care coordination
      • Innovative clinical programsare available to employees and their families, such as:
        • Healthy Pregnancy Program
        • Congenital Heart Disease (CHD) Resource Services
        • Transplant Resource Services

HealtheNote Reminders educate members about avoiding a health event

  • through mailings that emphasize prevention, early detection and treatment.
  • Health and Wellness tab on myuhc.com® encourages self-care goals, inspires healthy action and offers motivation through online health programs, tools and information.
  • Integrates with other UnitedHealthcare programs and services including behavioral health, wellness and optional clinical programs.
